Sante Initiative Uganda delivers an integrated, technology-enabled community model that combines vision screening, NCD screening, affordable assistive products, health education, referral linkage, livelihoods support, and environmental sustainability interventions for underserved communities in Uganda. Implemented through market hubs, taxi parks, schools, savings groups, VHTs/CHWs, community associations, and other trusted local platforms, the model uses digital tools to support screening, data capture, referral tracking, follow-up, and community health insights. By connecting preventive health with economic empowerment, climate-conscious community action, and digital innovation, Sante is building healthier, safer, more productive, and more resilient communities.

 

1,340 Lives Impacted

Sante Initiative Uganda is improving health, livelihoods, productivity, and community resilience by bringing integrated, technology-enabled services closer to underserved populations. Through market hubs, taxi parks, schools, savings groups, VHTs/CHWs, and community associations, Sante provides vision screening, affordable eyeglasses, NCD risk checks, health education, counselling, referral support, and follow-up for people who are often missed by formal health systems.

The model strengthens livelihoods by helping adults see clearly, work safely, remain productive, and access affordable health solutions that reduce preventable household costs. Through savings groups and community structures, Sante links health access to economic empowerment, especially for women, youth, caregivers, and informal workers. Sante also uses digital tools to improve screening quality, capture community-level health data, track referrals, support follow-up, and generate insights that can inform planning and partnerships. In line with its climate and environmental sustainability agenda, Sante is positioning community platforms to promote cleaner, safer urban spaces, waste-conscious practices, and youth- and women-led green livelihood opportunities. To date, Sante has reached 1,340 people through community outreaches, distributed affordable eyeglasses, identified clients needing further care, and strengthened community entry points for health, livelihoods, digital innovation, and resilience.

 

The Innovation

Sante Initiative Uganda’s innovation is an integrated, technology-enabled healthy communities model that combines preventive health, vision care, NCD screening, referral linkage, livelihoods support, environmental sustainability, and digital data systems. The model is implemented through trusted platforms such as VHTs/CHWs, market hubs, taxi parks, schools, teacher associations, savings groups, community outreaches, and local partners. It reaches low-income urban, peri-urban, and rural populations, including informal workers, caregivers, teachers, older persons, women, youth, adolescents, and communities facing barriers of cost, distance, limited awareness, unsafe environments, and weak referral systems.

Sante’s uniqueness lies in connecting health access with economic, environmental, and digital transformation. Vision screening and NCD checks are used as practical entry points for broader health education, affordable assistive products, referral follow-up, digital screening, community data capture, savings group linkages, and livelihoods strengthening. The model also creates pathways for climate-conscious community action, including cleaner urban spaces and youth- and women-led green enterprises. As Sante scales, it aims to expand across districts, train CHWs/VHTs, strengthen digital screening and referral systems, support savings groups, contribute to national health data systems, and reach over one million people by 2030.
